LIST OF MEDICATIONS BEING REMOVED FROM 2012 FORMULARY

Thank you for allowing SilverScript Insurance Company to provide your Medicare Part D prescription drug coverage in 2012. Our clinical team works closely with medical providers so that drug therapy is safe, effective, and provides the best value for our members.

Changes are made to our formulary each new plan year. Below is an abridged Formulary Alternatives list for medications removed from the formulary in 2012 for each of our plans. Please also look at the 2011 Abridged Formulary included in your Annual Notice of Change (ANOC) packet to determine if you are taking any medications that have been removed.

Please print this page and take it to your doctor with your new formulary to obtain a prescription for a formulary approved alternative. By moving to a formulary drug, you will avoid paying full cost for a non-formulary medication.

This is not a complete list of all formulary alternatives covered by the Part D plan. The medications presented are for comparison purposes and may differ in effectiveness, dosing, side effects and/or drug interaction profiles. Always seek the advice of your doctor.
